2006 Preferred Provider List of Home and Hospice Care The New Mexico Association for Home and Hospice Care serves its membership through advocacy, networking, education and communication by promoting success and best practices for home care, hospice and related home health support services.
The New Mexico Community AIDS Partnership (NMCAP) supports community-based projects and organizations around New Mexico that provide HIV/AIDS prevention, education, harm reduction and direct services. NMCAP is sponsored by the National AIDS Fund and has been a statewide initiative of the New Mexico Community Foundation since 1993. Medicare Part D

New Mexico Department of Health HIV Prevention The New Mexico HIV/AIDS Prevention Program is a component of the Infectious Disease Bureau, Public Health Division, New Mexico Department of Health. Another component, the HIV/AIDS Services Program, operates a network of five Health Management Alliances located in each Health District of the State to provide medical and social services for people with HIV/AIDS. Southwest Care (Santa Fe area) Southwest C.A.R.E. Center is located in Santa Fe, New Mexico, and is one of five Health Management Alliances (HMAs) that serve HIV-positive residents of New Mexico. Southwest C.A.R.E. Center primarily serves Santa Fe and eight additional northern counties of New Mexico. The mission of Southwest C.A.R.E. Center is to provide HIV-infected New Mexicans with state-of-the-art medical care and needed psychosocial services in a complete, timely, confidential, respectful, and cost-effective manner, regardless of ethnicity, race, religion, disability, gender, socioeconomic status, geographic location, sexual orientation, or language; and to provide innovative and effective HIV-prevention services to at-risk populations. New Mexico AIDS Services (Albuquerque area) New Mexico AIDS services provides services for HIV-infected individuals in the Albuquerque metropolitan area. They provide case management, couseling, housing, and food bank services. Camino de Vida Center for HIV Services (Las Cruces area) Camino de Vida Center for HIV Services is a non-profit organization dedicated to providing comprehensive care to persons impacted by HIV/AIDS and to improving the health and awareness of the community through HIV/AIDS prevention. Camino de Vida is part of the New Mexico Department of Health's regional HIV Health Management Alliance, serving Catron, Doña Ana, Grant, Hidalgo, Luna, Otero and Sierra counties. We provide comprehensive quality care to those impacted by HIV/AIDS. The challenges of living with HIV/AIDS are changing everyday. New drug therapies and prevention efforts mean that many people are living longer and healthier lives. Our goal is to ensure those impacted by HIV/AIDS receive care, education and support. Pecos Valley HIV/AIDS Resource Center a.k.a. Alianza of New Mexico (Roswell area) Pecos Valley H.A.R.C. is located at 808-B N. Union Avenue in Roswell, New Mexico and is one of five Health Management Alliance (HMA) members that serve HIV-positive residents of New Mexico. Pecos Valley H.A.R.C. serves the following nine southeast New Mexico counties: Chaves, Curry, De Baca, Eddy, Guadalupe, Lea, Lincoln, Quay and Roosevelt. The AIDS InfoNet is a project of the New Mexico AIDS Education and Training Center in the Infectious Diseases Division of the University of New Mexico School of Medicine. The InfoNet was originally designed to make information on HIV/AIDS services and treatments easily accessible in both English and Spanish for residents of New Mexico. It has become an international resource for information on HIV/AIDS. Two-Seven (HIV and African Americans in New Mexico) Two-Seven is an event organized by a coalition of agencies who organize an annual event that occurs each February 7th in Albuquerque to increase HIV/AIDS awareness among African Americans in New Mexico. The major focus of 2/7 is the annual conference that fosters awareness of the nature of HIV and AIDS and its impact on our [African-American New Mexican] community.
